Joint manipulation and mobilization are advanced, hands-on physiotherapy techniques designed to improve joint mobility, reduce pain, and restore proper movement patterns. Ideal for individuals experiencing stiffness, joint dysfunction, or restricted range of motion, these treatments are commonly used for conditions such as arthritis, neck or back pain, shoulder impingement, and post-surgical stiffness. A lot of manual therapy starts and ends with finding where you're stiff and pushing on it. FCAMPT-level assessment is different — it starts with identifying exactly which joint, at which segment, is actually restricted, and why. That distinction is the difference between treatment that works and treatment that feels good for an hour.

What FCAMPT-level assessment finds that generic manual therapy misses:

Most manual therapy assessment is regional — "your neck is tight," "your back is stiff." FCAMPT training goes a level deeper: segmental assessment, meaning we identify the specific spinal level or specific joint that's actually restricted, rather than treating the general area. A patient might present with diffuse low back pain, but the actual restriction driving it is a single segment — L4-L5, for example — that's been compensated around by every joint above and below it. Treating the whole region without identifying that specific segment means you're treating symptoms, not the source.

Mobilization vs. manipulation — how we choose:

At MSKrehab, mobilization is the default approach for most conditions. It's a slower, more controlled, graded movement of a joint, and it produces comparable results to manipulation for the majority of presentations without the higher-velocity force. For the upper cervical spine specifically, mobilization is used rather than manipulation — the upper neck carries different risk considerations that don't justify a higher-velocity technique when a graded approach gets you the same outcome.

Manipulation can have a specific role:

A joint that's genuinely "stuck" — not moving through its expected range despite mobilization — can respond well to a manipulation in the short term. It's a tool for a specific finding, not a default treatment. If your joint responds to mobilization, that's what we'll use.

Conditions we commonly treat with manipulation and mobilization:

  • Spinal joint restriction contributing to back and neck pain

  • Shoulder, hip, knee, ankle, and foot joint stiffness following injury or surgery (if applicable)

  • Joints that have become restricted from compensation around another injury

  • Post-MVA joint restriction (see MVA rehabilitation)

  • Chronic spinal stiffness that hasn't responded to general stretching or generic manual therapy

What this looks like in your assessment:

You'll get a thorough history and a hands-on segmental examination before any treatment. We're not guessing which joint to work on — we're testing it specifically. If mobilization resolves the restriction, that's the plan. If a joint remains stuck despite mobilization, manipulation may be used as part of the plan, followed by reassessment to confirm the joint has actually changed.

Manipulation and mobilization are rarely standalone treatments at MSKrehab — they're typically combined with soft tissue and fascial therapy for the surrounding muscle tissue, Gunn IMS where nerve-related muscle shortening is contributing to the restriction, and exercise prescription to maintain the joint mobility gained in session. A joint that's mobilized but never loaded properly afterward tends to drift back toward the same restriction.
