It is a clinical assessment process that identifies your specific pain triggers, determines your current spinal stability capacity, and builds a rehabilitation program from those findings — not from a protocol designed for an average patient.

Who is the McGill Method is designed for?

This approach is particularly effective for patients with chronic low back pain who have not responded to generic physiotherapy or exercise programs. This includes disc herniations, degenerative disc disease, facet joint pain, and complex presentations where previous treatment has provided temporary relief at best.

The Assessment Process

McGill Method assessment identifies your directional tolerance — the specific movements and postures that load your spine and provoke symptoms — and your baseline spinal endurance capacity using validated clinical tests. This tells us exactly which movements to avoid while your spine stabilizes, and which exercises are appropriate at your current capacity. Patients are frequently surprised by what they can actually do when the right foundation is established.

Why generic programs fail low back patients:

The most common reason patients with chronic low back pain don't improve with exercise is that they've been given exercises their spine can't yet tolerate, or that they've been told to "strengthen their core" with exercises that load the spine in ways that perpetuate their symptoms. Sit-ups, crunches, and aggressive loaded flexion are common examples. The McGill Method specifically identifies and avoids provocative loading during the early rehabilitation phase, building stability from the ground up.

Integration with Other Treatments

For patients with a neuropathic component — nerve irritation driving muscle shortening and inhibiting the activation needed for spinal stability — Gunn IMS dry needling is commonly used alongside McGill Method rehabilitation at MSKrehab. FCAMPT manual therapy/clinical reasoning and Joint Manipulation and Mobilizations addresses segmental spinal dysfunction identified in the assessment.
